What is Maternity Care Coordination and who is eligible?
Carteret County Health Department’s Maternity Care Coordination (MCC) is a voluntary program which provides supportive services to pregnant women who are eligible for Medicaid. We recommend beginning services as early as possible in the pregnancy to help ensure the best possible health outcomes for both mother and baby.
How do I enroll in Maternity Care Coordination?
To enroll, the client meets with the Maternity Care Coordinator to identify areas of need or concern. Together, they create a plan to address the woman’s specific needs. The Maternity Care Coordinator stresses the importance of going to all pre-natal medical appointments. Follow-up is by phone, home visits, at the Health Department and other community settings. The Maternity Care Coordinator continues to stay in touch through the time of delivery of the baby and for two months after.
What is the referral process for Maternity Care Coordination Services?
The Health Department welcomes referrals to the Maternity Care Coordination Program from medical offices, hospitals, social services, schools, and through self-referral.
