What are Community Based
Services??
Not all services provided by
the Carteret County Health Department are delivered onsite
through our offices. Some services are designed to be delivered
in the home or in the community. Sometimes these services
consist of community members meeting to identify service
gaps or community education needs.
In the Post Partum/Newborn Care
Program, visits by a registered nurse to newborns and their
mothers in their home setting are made within 2 weeks of
hospital discharge. The purpose of this visit is to provide
education, information and support to the new mother. Resource
information and referrals may also be offered.
Home visits by a registered
nurse are also made to high risk pregnant women in their
home for education, counseling, assessment, clinical and
emotional support. These women referred by their physician
and meet specific high risk eligibility. Conditions that
may make a pregnancy high risk could include diabetes, hypertension
or pre-term labor.
Community based services also
include Sudden Infant Death Syndrome (SIDS) Counseling.
A trained SIDS counselor is available for families who have
lost a child from SIDS. Community presentations on ways
to prevent SIDS such as “Back to Sleep” (positioning
the baby on their back instead of their stomach or side)
are available for families and child care providers.
