What are Case Management Services?

Case Management Services are those services provided by a care coordinator from the health department who is assigned to you or your family to help you identify and find services and resources.

Child Service Coordination is a program for families with children from birth to 5 years of age at risk for developmental delay or with a diagnosed condition or special need. Children can be referred by any source. The family is assigned a child service coordinator who through home visits, telephone calls and other personal contacts will:
    Identify programs, services and resources that meet family
   needs;
    Provide developmental screenings;
    Serve as a link between you and your child's doctor or nurse;
    Suggest ways you can strengthen parent-child relationships;
    Introduce you to parent support programs if available; and
    Offer you encouragement and support.

Maternity care coordination is for pregnant women who qualify for Medicaid. Women are assigned a care coordinator who will:
    Help you identify your strengths and needs through an assessment of medical, nutritional, educational, financial, and psychosocial factors;

 





 

    Assist you in arranging for services that are appropriate and provide continuity of care;
    Assist you in meeting your special needs such as prenatal care, transportation, adequate housing,
   education, substance abuse counseling, smoking cessation, labor and delivery education, preparing
   for your baby, breast feeding information, and referrals to other resources and information.

Who is eligible for Case Management Services?

For Child Service Coordination, children from birth to 5 years and who are at risk for developmental delay or with a diagnosed condition/special need. There is no charge for these services.

For Maternity Care Coordination, women who qualify for Medicaid. There is no charge for these services.
